Nutrition
Cornell Cooperative Extension of Jefferson County’s Nutrition Program offers free or low-cost workshops and resources focused on food safety, nutrition, and healthy eating on a budget. Our team provides engaging programs for all ages including youth activities for schools and community groups and our key initiatives like the North Country Fruit & Vegetable Prescription Program and the 1115 Waiver Program. Each program is designed to promote nutrition education and overall wellness across our community!
Our Nutrition Efforts
NC-FVRx – North Country Fruit and Vegetable Prescription Program
The North Country Fruit and Vegetable Prescription Program (NC-FVRx) helps reduce food insecurity and increase access to fresh fruits and vegetables. Participants are referred through healthcare providers or community organizations and attend six weekly nutrition workshops, offered in person or virtually. Each session includes practical lessons on nutrition, food safety, smart shopping, and label reading, often with a cooking demo. Participants receive $25 in produce vouchers per class, earning up to $150 total upon completing the program.

1115 Waiver Program
This new program is offered in partnership with Healthy Alliance to address social care needs within our community. We offer 3 different services, all of which are billable to Medicaid! Medicaid members enrolled in any plan are eligible to receive screening and navigation services that are billed directly to Medicaid. Medicaid members with managed care plans or those who are enrolled in enhanced services are eligible to receive our clinical nutrition services which are directly billed to Medicaid.
Screening Services
Screens for a variety of social care needs, including assistance with housing, transportation, care management, nutrition counseling and education and much more.
Navigation Services
Helps clients and families navigate through a wide variety of available social care services offered within the community. Referrals to the services are based on eligibility.
Clinical Nutrition
Our Registered Dietitian will provide nutrition education, healthy meal planning, diet-related disease management through nutrition intervention and more! Please note this service is only available to those on a Medicaid managed care plan.
